Before beginning to fill out your form, please gather and save each individual traveler’s documents as a separate file (a multi page file is also acceptable if that traveler has their documents broken up in more than 1 page).
Please name these files in a way that is easy to remember – e.g. Tzippy Proof of Judaism , David Government ID, etc…….
Save a CLEAR COPY if each travelers Government Identification to your computer or device where you can easily upload it to your form.
Acceptable Proof of Judaism is a letter from a recognized synagogue Rabbi from a Rabbinical Organization in North America, confirming how the Rabbi knows you and stating that you are Jewish and born to a Jewish mother.
